An additional fee of Rs 70/- towards surcharge will be charged

INCLUSION OF SPOUSE NAME IN PASSPORT

  • Filled in application form in Black Ink (in Block Capital letters)
  • Passport with photocopy of pages 1,2 and last page of the passport
  • Copy of valid Residence Permit
  • Copy of valid Work / Occupation Permit
  • Marriage Certificate copy with original
  • Two recent Passport sized colour photographs (See‘Guidelines for Photographs’)

  • Address proof (original and copy) like Electricity Bill or Telephone Bill

  • Filled in application form in Black Ink (in Block Capital letters)

  • Documents indicating stay of three years abroad OR copy of Matriculation Certificate and above alongwith original

CERTIFICATE

  • Passport with photocopy of pages 1,2 and last page of the passport

  • Letter from local employer / Board of Investment

MARRIAGE
CERTIFICATE

  • Filled in application form in Black Ink (in Block Capital letters)
  • Passport of both husband and wife with photocopies of pages 1,2 and last page of the passport
  • One recent Passport sixe photograph each of husband and wife(See ‘Photograph guidelines’)
  • Letter from local employer / Board of Investment

DEATH
CERTIFICATE

  • Letter from company where deceased was working indicating details of last payment made to deceased and dues, if any (with copy of recent Pay Slip)
  • Passport of deceased with photocopies of pages 1,2 and last page of the passport
  • Copy of valid Residence Permit
  • Copy of valid Work / Occupation Permit
  • Copy of employment contract signed by the company and the deceased
  • Autopsy report indicating cause of death
  • Death Certificate issued by local authorities

NIL

EMERGENCY
CERTIFICATE

  • Letter from individual or company where he is working indicating reasons for requirement
  • Filled in application form in Black Ink (in Block Capital letters)
  • Copy of Indian Passport (if any)
  • Copy of Police FIR (if Passport is lost / damaged)
  • Copy of valid Residence Permit
  • Copy of valid Work / Occupation Permit
  • Four recent Passport sized colour photographs (See‘Guidelines for Photographs’)
  • Copy of ticket booking for travel to India

NRI
CERTIFICATE

  • Letter from applicant indicating his period of stay in Mauritius, his employment details and reasons for requirement of certificate
  • Passport with photocopy of pages 1,2 and last page of the passport
  • Copy of valid Residence Permit
  • Copy of valid Work / Occupation Permit

TRANSFER OF
RESIDENCE
CERTIFICATE

  • Letter from applicant indicating name of his employer, designation, date of arrival in Mauritius and likely date of departure from Mauritius
  • Passport with photocopy of pages 1,2 and last page of the passport
  • Copy of valid Residence Permit
  • Copy of valid Work / Occupation Permit

ATTESTATION

OF POWER OF
ATTORNEY

  • Document in original with photocopy (applicant and 2 witnesses to append their signature in the document in front of Consular Officer)
  • Indian Passport of applicant with copies of pages 1,2 and last page of the passport and 2 witnesses (Indian national)
  • Copy of valid Residence Permit of applicant and 2 witnesses
  • Copy of valid Work / Occupation Permit of applicant and 2 witnesses
